1. Primary Contact will be contacted immediately, and Secondary contact will be called if necessary.

2. Emergency services will be called if necessary

3. Basic first aid practices will be employed if necessary

Assumption of Risk

1. I am familiar with the active and physical nature of and possible risk of injury in participating in the classes.

2. All participants will be prepared with appropriate footwear (sneakers) and clothing in which they can move comfortably.

3. I agree to hold Eli Green and Jewish Music Toronto harmless from and against any liability for injury or damages that may be incurred as a result of classes.

4. It is my child’s right and responsibility to inform Eli Green of any discomfort, fatigue, or any other symptom that may occur in class.

5. I will remind my child to be an active participant, and that all applicable rules for participation will be followed at all times. If any negligence is created on the part of my child during the classes, Eli Green and Jewish Music Toronto will not be held responsible.

Payment for programs is required in full in order to finalize registration and enrollment. Written notice is required for all program withdrawals.
